Application/Nomination Instructions
All documents that will be uploaded with application form must be in PDF files; except for your headshot photo, it must be in a JPEG file.
a. You are only allowed to upload one PDF file per type of document upload. Please make sure you scan all your documents as PDF and save them as one single PDF file per type of document upload.
b. Give each document and title and name your files using the following naming format: ▪ last name, followed by your first name, the award category, and then the type of document. Examples: Angel.Ocampo_clinicalstaffnurse.photo Angel.Ocampo_clinicalstaffnurse.Personal.statement Angel.Ocampo_clicalstaffnurse.Nominator.statement Angel.Ocampo_clinicalstaffnurse.Contributions Angel.Ocampo_clinicalstaffnurse.Recognitions Angel.Ocampo_clinicalstaffnurse.Education
c. It is recommended to have a designated folder where you can save all your documents. This will facilitate the ease of uploading your documents.
1. Email Address: a. Provide the email address where you want to be contacted of the result of your application.
2. Personal Statement: a. The 500-word statement should address the quality of your professional persona relevant to the award category. Answer the question “What are the key/signature qualities that make me eligible for this award? b. Save and submit your essay as a single PDF file following the naming format of the examples above.
3. Nominator Statement:
a. Obtain your nominator’s statement that addresses the following: “In 350 word or less, please provide a summary/personal statement of the meritorious qualities of the applicant/nominee for the selected award. Address the significant, relevant, and outstanding contributions for the selected award category and sustained involvement with PNACF. “What is the key/signature qualities that make my nominee eligible for this award?”
b. Save and submit the document as a single PDF file following the naming format of the examples above.
4. Your Photo:
a. It is expected you be dressed in business attire in your headshot photo. Failure to comply with this requirement could result in disqualification.
b. Save and submit your photo in a JPEG file following the naming format of the examples above.
c. You will be required to sign a Photo Release Consent in the application form.
5. Contribution Section of your application
a. Gather and submit your chosen exemplars of contributions made within the timeframe stated within the criteria for your award category.
b. Scan and save documents/research papers/publications (articles in peer-reviewed journals, chapters in reference books/textbooks, poster presented, and newsletters (from a professional organization, hospital/unit, etc.) verifying contributions relevant to your award category.
c. Include current contributions to Professional Organizations/Affiliations such as ANA and other professional nursing organizations.
d. Note: The Awards Committee is aware that we have not provided exhaustive examples of types of contribution documentation. You will want to consult with your nominator to determine what you want to include that will best demonstrate the criteria and align with your personal statement to position you as a competitive applicant in your chosen award category.
e. Scan and save all relevant award category-oriented documents verifying nursing contributions into a single PDF file. following the naming format of the examples above.
6. Recognition section of your application
a. Gather and submit your chosen exemplars of recognition made within the timeframe stated within the criteria for your award category.
b. Note: BLS, ACLS, PALS, etc. are not acceptable documentation of recognition.
c. Your professional certification/s should be current.
d. Nominations to an award in lieu of actual recognition awards are taken into consideration.
e. Scan and save copies of your certificates of award/recognition (or any form of document verifying your achievements).
f. Note: The Awards Committee is aware that we have not provided exhaustive examples of types of contribution documentation. You will want to consult with your nominator to determine what you want to include that will best demonstrate the criteria and align with your personal statement to position you as a competitive applicant in your chosen award category.
g. Scan and save all relevant award category-oriented documents verifying nursing recognition into a single PDF file following the naming format of the examples above.
7. Continuing Education section of your application
a. Gather and submit supporting relevant continuing education documents within the timeframe stated within the criteria for your award category.
b. This includes being a presenter/speaker at professional meetings. provide supporting documents).
c. Save the document/s as a single PDF file following the naming format of the examples above. Scan and save copies of your proof of membership (e.g., Current Member ID Card) as a single PDF file following the naming format of the examples above.
8. Confirmation of Information:
a. You will be required to attest that the information you provided on the application is true to the best of your knowledge.
b. Send our application and all required documents to the Awards and Scholarship Committee Chair: Aurita Panggat at aqpanggat@aol.com and cc’d to the Co-Chair: Alicia Tullo at profatullo@gmail.com
c. Upon receipt, you will receive a confirmation at the email address you provided on the application. This is the same email where you will be contacted of the result of your application.
